Understanding aviation accident causes

Accidents rarely reduce to one isolated mistake or failure. Cause categories are useful summaries only when the initiating event, contributing conditions, and investigation findings remain distinct.

Cause is not the same as sequence

A factual sequence might begin with weather, continue with a system failure, and end with a crew response. The final report may identify several causal and contributing factors. A broad archive category highlights the dominant supported pattern; it does not reproduce the investigation's complete analysis.

Mechanical failure

A supported failure of an aircraft structure, engine, system, or component. Maintenance and design factors may still contribute.

Weather

Conditions such as icing, storms, turbulence, wind, visibility, or atmospheric effects that materially shaped the event.

Human factors

Crew, controller, maintenance, organizational, or decision-making factors supported by the record—not a shorthand for blame.

Criminal act

Sabotage, hijacking, shootdown, or another intentional act established by credible evidence.

Unknown or disputed

The evidence is insufficient, the investigation did not reach a supported conclusion, or reputable sources remain materially inconsistent.

Avoid hindsight and single-factor explanations

Investigations evaluate the information and constraints present at the time, not only the outcome. A crew action that appears decisive afterward may have followed ambiguous indications, incomplete procedures, system behavior, or organizational pressure. Responsible research preserves that context.

Read the underlying record

Use cause archives to find related events, then open individual records and authoritative reports before making comparisons. A category is a navigation aid, not a substitute for evidence.
